Workshop Participation Consent Form

Important Notes:
  • Each participant must fill out a separate form.
  • All participants under 18 years old must have parental/guardian consent.
  • For participants under 18, staff will contact the parent/guardian listed under the Parent/Guardian’s Information section for verification.
  • For example, if a parent is attending with a child, two individual forms must be submitted.
  • We reserve the right to refuse entry or participation if these requirements are not met.

4. Acknowledgment of Risk

I, the undersigned, understand that participation in this Workshop may involve certain risks and hazards, including but not limited to use of tools, materials, or equipment. I acknowledge that the workshop organizers will make reasonable efforts to ensure safety.

5. Indemnity and Release of Liability

I release [Workshop Organizers/Community Workshop @ VidaCity] and affiliates from liability for any claims or damages from workshop participation, including negligence. In an emergency, I authorize workshop personnel to seek necessary medical treatment on my behalf. I agree to be responsible for any medical expenses incurred.

6. Media Release & PDPA Consent

I consent to [Workshop Organizers/Community Workshop @ VidaCity] using visual/audio recordings of me for promotional or educational purposes.

By signing below, I consent to [Workshop Organizers/Community Workshop @ VidaCity] collecting, using, and disclosing my personal data for purposes related to organizing and managing the workshop. I understand that I can withdraw my consent at any time by notifying the organizers in accordance with our PDPA Policy.
